What moves it
Each hour multiplies by the cabin class rate. London scales sharply against a regional sector for that reason alone.
July to early September and December to New Year. Rates stay inside the bands, but the choice of aircraft narrows — large cabins first.
November to March, with January and February seeing the most reroutes. Rain does not raise the rate; holding, alternates and crew duty do.
Multi-stop itineraries are built sector by sector, with repositioning and overnight fees on top.

Priced differently
Medical configuration, onboard staff, stretcher installation and hospital coordination are line items of their own, on top of flight hours.
Most efficient folded into a trip you are already making. Indonesian quarantine and veterinary rules apply, and carrier handling is billed at both ends.
Repeated sectors — a resort or corporate group running Singapore–Bali — are usually better on block hours than on one-off quotes.

Because pricing is time-based. Each hour multiplies by an hourly rate running from about USD 2,000 for a turboprop to USD 17,000+ for ultra long range. A London sector is not a longer version of a Jakarta one — it is a different order of cost.
It depends on passenger count, how sensitive the schedule is, and the mission. For executive teams, families with awkward connections, or medevac and pet relocation, the control over routing usually justifies it. For one flexible traveller, a premium commercial cabin is more economical.
Aircraft, crew, fuel for the planned routing, standard catering and standard handling at both ends. Positioning flights, de-icing, out-of-hours surcharges, upgraded catering and any medical or pet services are extra and should appear as separate lines.
